The following forms must be completed by all student-athletes wishing to participate in intercollegiate athletics.

Prior to participating on a team from Wesley College, athletes must provide the Sports Medicine Department with current address, emergency contact, insurance, medical alert and health history information and other necessary forms. To expedite this process the Sports Medicine Department uses an online data entry system through CastleBranch.

For freshmen/transfers: 

To submit medical information required to participate in athletics, follow the instructions given to you by Wesley College Admissions Office to place an order on CastleBranch at mycb.castlebranch.com and using the appropriate packaging code for student-athletes in the space next to “place order.” From there, follow the instructions to upload your immunizations, sickle cell test results, and insurance card. After June 1, you will need to go to download and complete the physical form packet with your doctor and also fill out demographic information such as - address, emergency contacts, primary insurance information, medical history and precautions, medical forms, and the concussion fact sheet. The email you use when placing your order will become your username for myCB and will be the primary form of communication for alerts and messages. KEEP TRACK OF YOUR LOGIN INFORMATION AND CHECK YOUR EMAIL in case any of your submissions are denied and need to be uploaded again. For returning student-athletes:

Returning athletes are to go back onto their CastleBranch account where they will find a link to download and complete the renewed physical exam (#11 in their checklist). That form will include the returning athlete physical exam, medical forms for sports med, and areas where they can add or change any demographic info if needed.

All online paperwork for the upcoming academic year should be completed and submitted after June 1.
