Student-Athlete Forms

The following forms must be completed by all student-athletes wishing to participate in intercollegiate athletics.

Student-Athlete Publicity Form (This form is for student-athletes attending Wesley.  If you wish to contact a coach to be recruited, please click on the RECRUIT ME link at the top of the page.)

The following four forms should be completed and mailed to the Wesley College Sports Medicine Office at:

Paperwork for the upcoming season should be filled out and submitted after June 1.
